Signed Written Thesis/Dissertation Approval Form
This form should be signed by your committee after you have passed your final defense and made the requested changes for your committee.
- Thesis (M.S., M.A.) Form
- Dissertation (Ph.D) / Record of Study (Ed. D.) Form
- Doctor of Engineering, Record of Study (D. Eng.) Form
Note: Two separate 'approval forms' should be prepared for your oral defense - 1) the Written Thesis/Dissertation Approval Form (prepared by the student) and, 2) the Report of the Final Examination Form (generally this is prepared by OGS and provided to your department). The Written Thesis/Dissertation Approval Form should be returned to the Thesis Office as soon as the thesis/dissertation is approved by the committee and all signatures have been obtained. The Report of the Final Examination Form should be submitted to OGS within 10 days of your defense.

*Note: Completion of survey questions is highly recommended but optional. Each student must electronically sign the consent page, review the survey questions, and submit the final certificate/email to the Thesis Office which includes your name. Please refer to the agreement form for additional information.
